I actually wonder how common this is. We've seen one other picture like this so far. Think about all the times when no one had a camera to take photo evidence of the tragedy. It seems like this should be pretty common based on ladder design, though I don't remember the last time I was on a ladder and worried about it happening. I guess that's how the best accidents happen.
